PASSPORTS and ADDITIONAL VISA PAGES

Full and limited validity passports are issued by the American Citizens Services Unit.   In Thailand, persons seeking a United States passport must appear in person at the Consulate to apply. To make an appointment for a passport service, please visit the appointment page.

For further information you may refer to the United States Department of State website on passport applications. Please remember that some procedures for passport applications abroad may vary according to the country in which the passport application is made. The approximate turnaround time for new passports is approximately 4 weeks from the date of application; when applying, you will receive information regarding passport pick-up from the officer.

DOCUMENTARY REQUIREMENTS

1. Proof of Citizenship (Please bring one of the following original documents):

  • Previously issued United States passport
  • United States birth certificate (original or copy certified by an official government custodian of birth records)
  • Certificate of Naturalization
  • Consular Report of Birth Abroad 
     

2. Proof of Identity (Please bring one or more of the following original documents):

  • Previously issued United States passport
  • Certificate of Naturalization
  • A formal identity document issued by a competent authority of the municipality, state, or country in which the applicant resides (e.g. driver's license)

 
3. Photographs

Two identical photographs are required. They must be recent, 2 x 2 inches (with the size of the head between 1 and 1 3/8 inches), and they must be in color.  The photos must be clear, front view, full face, and in focus. The background must be plain white or off-white. Please print the applicant's first and last names on the backs of both photos. FEES

The Consular Section accepts cash payments in United States Dollars or Thai Baht, as well as credit card payments.  Unfortunately, we cannot accept checks.   Current fees for passport services are as follows:

Minor Passport (Under Age 16)                       $85.00

First Adult Passport (Age 16 & Older)             $100

Replacement for Lost and Stolen Passport     $100

Renewal Adult Passport (Older than Age 16)  $75

We encourage all customers to complete any forms prior to visiting the Consulate on the day of their appointment.  Commonly used forms may be accessed electronically below.  Please do not sign any form until asked to do so at the Consular Section.

APPLICATIONS FOR MINOR CHILDREN

For children under 16 years of age, the consent of both parents is mandatory. If one parent is not present in the country of application, he or she may complete and notarize Form DS-3053 and send this form to the other parent, thereby granting permission for the application and issuance of a passport to the child. In addition, a certified birth certificate and photocopy are required as evidence of the child's relationship to the parents/guardians.

Passport applicants who are United States citizen minors under the age of 16 must appear personally with their parents to establish identity, proof of citizenship, and proof of relationship.  For parental application permission, both parents must appear together and sign, or one parent may appear to sign and submit the other parent’s notarized statement, or one parent may appear, sign, and submit primary evidence of sole authority.  Minors ages 16 and 17 may apply in person with their own identification, but for security reasons, parental consent may be required to accompany the child and parent identification.

For children under the age of 5, parents should be prepared to provide photos of the child at each year of age to facilitate the Consular Officer's identification of the child from infancy.  The Consular Officer may request this information of older applicants as well.

LOST OR STOLEN PASSPORTS

1. Applicants must present the original Thai police report concerning the circumstances of the loss or theft.

2. Applicants must complete an Affidavit of Loss or Theft (available at the American Citizen Services Office).

3. Applicants are normally required by the Royal Thai Government to obtain a replacement entry stamp in their passport.  The American Citizen Services section will provide information on how to accomplish achieving a replacement entry stamp.

EMERGENCY PASSPORTS

Emergency passports are issued on a case by case basis, and are valid for one year only.  Emergency passports require a minimum of 24 hours to process. 

EXTRA VISA PAGES

To obtain additional visa pages, you must complete an Application for Additional Visa Pages (DS-4085: Amendment/Validation Application).  Please complete the form using black ink and submit it along with your passport to the American Citizen Services section at the United States Consulate.  Additional Visa Page requests are usually completed the same day as application at no cost, while you wait.  Please allow one hour for this process.

PASSPORT EXPIRATION DATES

Please be aware that many countries require six full months of validity remaining on a passport in order to issue a visa on arrival. When traveling, please remember to carry a photocopy of your passport separately from your passport. In the event of loss or theft, the copy will facilitate issuance of an emergency passport.
